This Microsoft Access database relates most specifically to units in case management and managing information, and includes referrals and case notes. Jim Longbottom wrote it and it is supplied by LayUp Sports and Leisure Training Centre. It was specifically developed for use in assessment of ACAS's Certificate IV in Youth Work.
It has two macros, which your anti-virus program might warn you about. Click to ignore the warning. (One macro is simply to go to the end of the record so you don't overwrite the first record. The other macro is simply to make the report page come to full screen size.)
The file is fairly large (2.6 mb), so downloading might take a while. You'll need the password to open it; it's "WCCI". Save the file on your own system and change the password.
